What is the procedure for setting up counseling at Haven Center?

To begin counseling at Haven Center, there is a 4-step process that is followed.

(1): Call Haven Center at 651-277-4283 between 10 am and 4 pm, Monday – Friday. One of our administrative staff will take your contact information, and discuss how you would like to pay (also taking insurance information if applicable). You can also come in to provide this information. Any information provided via email cannot be guaranteed confidentiality.

(2): Haven Center will authorize your payment information. This can take a couple of business days. Note: For certain types of insurance, you may be placed on a waiting list before a counselor is available who can accept that insurance. A person who is willing to do self-pay (sliding fee scale, based on annual household income) may be able to get in more quickly.

(3): The Intake Appointment usually can be set up at the time of your first phone call, and can be completed as soon as your payment system is authorized, usually within a week of your initial contact with Haven Center. When coming to the intake appointment, please bring your insurance card and a credit or debit card to keep on file. It will also be important to arrive early to begin completing the necessary forms (intake questionnaire and the client data form) located at the corner above the coffee table. These are also available in the Forms section of our site. If you would like to complete them early, and bring them with you. Complete either the Adult or Child/adolescent Questionnaires, and also the Client Data Sheet. It will be determined at the intake which counselor best fits your individual needs for ongoing counseling.

(4): The intake worker will schedule your first counseling session for you. After that, you will set up your appointments directly with your counselor. Each counselor has a business cell phone with confidential voice mail that clients may call to set up or change appointments, so be sure to get their phone number during your initial conversation with your counselor.

What documents should I bring with me on my first visit to Haven Center, in addition to Haven Center questionnaires?

(1): Clients using insurance should bring their insurance card with them, so that a copy can be made of it for Haven Center records.

(2): Haven Center requests credit or debit card information on each client for two reasons: (1) You may choose to use your credit or debit card to pay automatically from your account for your sessions. People using this method will be given a $5.00 discount per session. (2) We use these cash cards to cover late fees, if necessary.

(3): Clients paying on a reduced – sliding fee scale rate (scholarship – lower than regular sliding fee scale rate) will be asked to provide proof of annual household income for the previous year, and also for the current year. Proof of income for the previous year is the first 2 pages of the federal income tax form of each household member earning income for the previous year. Proof of income for the current year is best provided by copies of paycheck stubs for one month of the current year by each household wage-earner. Self-employed individuals provide current year documentation of income over the previous quarter (discuss with Haven Center how best to provide this).

How much notice do I need to give in order to change or cancel an appointment?

Haven Center requires 24 hours notice for changing or cancelling an appointment, otherwise a late fee is charged which does not come out of the client’s insurance. We charge the customary late fee rate in this region, which is $75. This is the same charge for clients using the self-pay option.

What number do I call in order to change or cancel appointments?

Your counselor or coach will give you their work cell phone number at your first appointment, or when they first talk to you by phone. That is the number you should use to cancel or change appointments. Be sure to keep the number with you at all times.

What do I do if I misplace my counselor or coach’s work phone number?

If you misplace your coach’s or counselor’s phone number, call the main office at 651-277-4283 and we will be happy to give it to you again, so that you can continue to communicate directly with your counselor or coach.

You may either use insurance for counseling (we accept most major insurance options) or Haven Center’s self-pay/sliding fee scale option for counseling. Life coaching is slightly less expensive, but insurance doesn’t cover it – that would have to be self pay.

Our self-pay options are based on annual household income. Haven Center’s rates for counseling are competitive with other counseling services in this area. The full rate is $130 per 45 minute session, if your annual household income is over $120,000 per year (1st tier). The very first session is slightly more expensive, because of the additional time involved in paperwork ($190)

We have 6 tiers of payment, based on annual household income. For example: te 5th tier is for people with income between $10,000 and $30,000 ($60 for the first session, $40 ongoing). The 6th tier is for people with income between $0 and $10,000 ($30 for first session, $20 ongoing). Proof of income for the previous and current year is usually required (ask for details).

Life coaching — which does not deal so much with emotional issues but with fine-tuning one’s lifestyle — is less expensive than counseling, but is also offered for self pay on a sliding fee scale. Cost of life coaching for the first tier ($120,000 annual income) is $100 per session. Cost of life coaching for the 5th tier is $30 per session, and for the 6th tier is $30 per session. At this time insurance does not cover life coaching in Minnesota.
